Use of the ALS-FTD-Q

Clinical use
For: universities, foundations, health professionals, hospitals, clinics, public health institutes
The ALS-FTD-Q can be used, multiplied and distributed without permission. Patients will receive the questionnaire free of charge.
Written permission and a licensing agreement is necessary if funding from commercial entities or the pharmaceutical industry is received.

For: commercial entities and pharmaceutical industry
The ALS-FTD-Q can be used, multiplied and distributed if prior written permission and a licensing agreement are obtained.
Patients will receive the questionnaire free of charge.

Educational use
